Kindergarten registration set at Crawford Central

MEADVILLE — Dates have been set to register for 2014-15 kindergarten classes in the Crawford Central School District.

Register from 7:30 a.m. to 4 p.m. at any elementary school on the following dates: April 7, Cochranton; April 8, West End; April 9, Second District; April 10, Neason Hill; and April 15, First District.

Your child must be 5 years old before Sept. 1, 2014, to register. At the time of registration, you must provide proof of residency. A valid Social Security Card is requested but not required for registration.

The child’s original birth certificate or hospital verification must be presented at time of registration. If a birth certificate is not available, forms for obtaining one for Pennsylvania born students may be obtained at the Crawford Central School Administration Office. If hospital verification is used, the child’s birth certificate must be on file in the school prior to entrance in the fall. No tests are given to children on the day of registration.

State law requires every child entering school for the first time to have immunizations. Call your physician or the Pennsylvania Department of Health (332-6947) to arrange an appointment for required immunizations.

The Crawford Central School District requires any student enrolling in the school district for the first time to have all mandated immunizations that are allowable at the time of entry (applicable to Crawford Central School District Policy No. 203).

If immunization cannot be given because of medical reasons, a statement from a physician will be necessary and should be taken to registration. A record of the child’s immunization and medical history should be available for the school nurse at registration.

Documentation required: Proof of residency; child’s original birth certificate; record of the child’s immunization and medical history.

Kindergarten registration forms are available at and may be completed prior to registration.
